The Fulbright Graduate Student Program Application
and Supplemental Forms available for the 2011-2012 Academic Year

Applications should be completed using the Interactive Word format and using the Online System.

All applicants should complete the Online Application Form
located at https://apply.embark.com/student/fulbright/international/

Please read the [Application Instructions] for more detailed information on completing the Online Application. If you have any further questions, please send an email to Inna Barysh, Program Coordinator, at student@fulbright.com.ua.

Original documents that require a signature (Letter of Reference, Signature Form) as well as a copy of your diploma must be sent to our office by regular mail.

Below is a WORD application that you can complete on your computer, print, and submit by mail as well as all supplementary forms.  The application must be also sent to the Fulbright Office electronically via e-mail student@fulbright.com.ua. Supplementary forms are available in WORD and PDF versions on this archive:

  • Letter of Reference
  • Report on Proficiency in English Form
  • Transcript Release Form (for post-secondary U.S. transcripts only)
  • Academic Records Information Form
You complete the Academic Records Information, Transcript Release and the Signature forms. The Letter of Reference and the Report on Proficiency in English Form are completed by other individuals. You can print the PDF version of these supplementary forms and forward them to the appropriate individuals for completion, or you can direct these individuals to this web site.
