Fulbright Faculty Development Program Application 2015-2016

By November 1st the candidates must submit the Online Application Form located at: https://apply.embark.com/student/fulbright/international/

In addition to completing the Online Application Form, applicants must also submit a packet of supplemental documents in order to be considered as an applicant:

Applicants are required to bring to the Fulbright office or submit by regular post next supplemental documents:

1)    Your references can complete the forms online. You must click on the ‘Recommendation' button on the Home page of this application to register your references. After registration your references will receive a link to the reference letter form by e-mail. You also need to submit the original letter of reference by regular post.

2)    You can download the Letter of Reference form and forward it to your references. The letter of Reference must be completed in English or in the original language with an English translation.  The references can then either mail the recommendations directly to the Fulbright office or release it to the applicant in a sealed and signed envelope, so the candidate can submit letters the of reference with the other documents.

  • University Transcripts – official, complete and certified academic documents covering the entire period of study at universities and other post-secondary institutions. Please note: copies of diplomas certified or notarized by a third party (i.e. public notary) will not be accepted as official. US schools only accept academic documents verified by the university's Educational Department, Director of Archives or International Office. Applicants must submit copies of the diplomas issued by non-Ukrainian institutions. Finalists will have to provide certified English translations of the academic documents (by official Translation Bureau or University International Office).
  • Report on Proficiency in English Form – optional form. Complete only the top section of this form and have the remaining portion completed by one of the certified English language evaluators listed on the form or a qualified teacher of English, who can chose any method for evaluating the applicant. Then evaluators can either mail the form directly to the Fulbright office or release itto the applicant in sealed and signed envelope, so the candidate can submit the document with the other documents.
  • Signature Form – mandatory form. Applicants must print out, complete, sign, and forward two original forms to the Fulbright Office.
  • Academic Records Information Form – for those applicants, who have already received their diplomas. A separate form should be completed for each college or university attended.
  • Transcript Release Form (for post-secondary U.S. transcripts only) - for those applicants, who have a diploma from the U.S. academic institution.
  • Portfolio – for candidates from creative fields (architecture, painting, film studies, music, sculpture, theatre, photography). Please submit up to 10 art works by e-mail (file size no more that 20 MB) to Inna Barysh at: fulbright.barysh@gmail.com. Please include your last name, program name "FFDP" and the document's name into the subject line of the e-mail, e.g.: Boiko_FFDP_Portfolio.

The submission deadline is November 1st annually.
